Pre-Examination Assessment

Pre-examination questionnaire needs to be returned within 24 hours to ensure efficient booking times. If the questionnaire is not received within 24 hours, we reserve the right to cancel the appointment.

On completion and acknowledgement of the terms and conditions, we will escalate your booking to our team of accredited examiners. You will receive an e-mail confirming your controlled office location if you have requested the test to be conducted at one in addition to  any further information once the form has been completed.

Your examiner will contact you directly should they require any additional information in preparation of the case. The finalising of questions and in depth consultation will take place in the pre-test interview at the actual test.

Please enter the first and last name of the client. The client is the individual who is organizing and funding the polygraph examination. Remember, the client may not be the person taking the test, but rather the one responsible for arranging and covering the costs of the examination.
Please specify the precise number of individuals who are scheduled to undergo the polygraph examination. The term 'examinee' refers to any person who will be subjected to the test. It is important to provide an accurate count of all participants who will be taking the lie detector test.
