Medical Conditions and Medications

Please read all sections of this agreement thoroughly and ensure you understand each point. Certain medical conditions may disqualify examinees from taking the polygraph examination, while others may require written consent from a doctor before proceeding.

Current or Past Medical Conditions: Do you or any of the examinees have any current or past medical conditions? Please specify details for each examinee. *

Prescribed Medication: Are you or any of the examinees currently taking prescribed medication? Please specify the type, dosage, and frequency for each examinee. *

Mental Health Diagnosis: Have you or any of the examinees ever been diagnosed with a mental health condition such as schizophrenia, bipolar disorder, or manic depression? Please provide details for each examinee. *

Antipsychotic Drugs: Are you or any of the examinees currently taking antipsychotic drugs? If taken in the past, when did you stop? Please provide details for each examinee. *

Examinees will not be permitted to undergo the polygraph examination if they have consumed alcohol, abused controlled substances, or taken recreational or non-prescribed drugs within 24 hours of the test.
If any examinee is pregnant or has given birth within the past 4 weeks, a doctor’s note is required prior to the examination.
For the examination, we require you to arrive as your ‘normal’ self, so request that you do not withdraw from any prescribed medication for the purpose of the test
Examinees with severe medical conditions such as epilepsy, heart disease, or other serious health issues that may interfere with the polygraph examination must obtain medical clearance from a doctor prior to participating.
Examinees are required to fully cooperate with the examiner, including answering all questions truthfully and openly. Non-cooperation or refusal to communicate can result in termination of the test.
Recent Surgeries or Injuries: Examinees who have undergone surgery or experienced significant physical injury within the past 6 months should consult a doctor before participating in the polygraph examination.

Pre-examination questionnaire needs to be returned within 24 hours to ensure efficient booking times. If the questionnaire is not received within 24 hours, we reserve the right to cancel the appointment.

On completion and acknowledgement of the terms and conditions, we will escalate your booking to our team of accredited examiners. You will receive an e-mail confirming your controlled office location if you have requested the test to be conducted at one in addition to any further information once the form has been completed.

Your examiner will contact you directly should they require any additional information in preparation of the case. The finalising of questions and in depth consultation will take place in the pre-test interview at the actual test.
