I understand that this examination is entirely voluntary. The examination will only proceed after the examiner has confirmed that the examinee is suitable for testing and the examinee has provided informed consent. The examinee retains the right to withdraw from the examination at any time, for any reason, without penalty, though applicable fees remain subject to the Cancellation & Refund Policy outlined below.

I understand that a booking administration fee of $100 is payable today to confirm my booking request. This fee covers preparation of the Pre-Examination Assessment, booking coordination, and related administrative services, and will be credited toward the total examination fee of for .

Once the examinee has completed the Pre-Examination Assessment and the examiner has confirmed the appointment details, the remaining balance is due directly to the assigned examiner within 48 hours, or within 24 hours if the examination is scheduled sooner.

If I wish to reschedule my appointment, I must provide a minimum of forty-eight (48) hours' written notice prior to the original appointment date.

The rescheduled appointment must fall within thirty (30) calendar days of the original booking date. Requests beyond this window will be treated as a cancellation, requiring a new booking and a fresh Booking Fee.

The Booking Fee is refundable only if the examiner determines, following their professional assessment, that the case or examinee is unsuitable for polygraph examination.

The Booking Fee is non-refundable in all other circumstances, including but not limited to: (i) cancellation by the client for personal reasons; (ii) failure to respond to contact attempts to finalize the examination or booking process; (iii) the examinee refusing to participate or failing to complete the Pre-Examination Questionnaire; or (iv) failure by either the client or examinee to cooperate with the examination or booking process.

If cancellation notice is received at least forty-eight (48) hours prior to the scheduled appointment time, the non-refundable Booking Fee may be transferred to a new appointment at no additional cost.

If cancellation notice is received less than forty-eight (48) hours prior to the scheduled appointment, or if the client fails to attend the appointment, all fees paid to the examiner are forfeited and cannot be applied to any future appointment.
